Posyandu (Integrated Health Post) is a form of basic health service that plays an important role in monitoring the growth and development of toddlers and the health of pregnant women. However, in practice, data recording and management at Posyandu Jorong Rogeh, Nagari Bukik Sikumpa, are still done manually, which causes various obstacles such as delays in recording, data duplication, and difficulties in reporting and searching participant history. To overcome these problems, this study aims to design a structured and integrated Posyandu information system with a modeling approach using UML (Unified Modeling Language). The method used in designing this system is the waterfall method, which includes user needs analysis and system representation through three main diagrams, namely UseCase Diagram, Class Diagram, and Entity Relationship Diagram (ERD). UseCase Diagram is used to describe the interaction between actors and the system, Class Diagram to define data structures and relationships between objects, while ERD represents a relational database structure that supports digital data management. The results of the design show that the designed information system is able to systematically map the entire Posyandu service process, including participant registration, recording examination results, and report generation. With this design, Posyandu is expected to increase efficiency and accuracy in data management, as well as become the basis for developing web-based or mobile digital applications.
